Collect a detailed course outline/class syllabi for the undergraduate subjects you have completed or are enrolled in, which meet the Melbourne DDS prerequisites:
human anatomy
physiology
biochemistry

Detailed course outlines are required. A one-paragraph description of a subject will not suffice.

Academic Transcripts

Your university can mail your final transcript directly to OzTREKK, or you can mail your transcript directly to OzTREKK, as long as it remains in its sealed envelope.

If you have completed studies from a number of post-secondary institutions, you will need to submit a final transcript from each institution at which you studied.

Applicants must submit their final transcript, which includes your final semester's marks and evidence that you have completed your degree.

If you are submitting a DAT Report, you need to have the Canadian Dental Association send an official transcript of your results to the OzTREKK office. If you haven't yet completed the DAT, you can order the transcript during your registration. If you have already completed the test, you will need to complete a request form via the CDA website, in order for the organization to mail your official results to OzTREKK. Test dates from January 2010 to June 2012 will be accepted for the 2013 intake. An official score report will need to be sent from the appropriate Dental Association - please contact the CDA or ADA, and make a request for them to send it to the OzTREKK office. For the 2013 intake, the timeframe for valid MCAT/DAT tests are those dated January 2010 to the present inclusive.

If you have written the Canadian DAT, please order a score report online. You can choose the University of Melbourne as the recipient, but please ensure that you input OzTREKK's mailing address as follows so that I receive the report directly:

OzTREKK has set up an understanding with the CDA, as they know that when students request for their transcript to be sent to our office, it's on the University of Melbourne's behalf.

Please note: If you are also applying to Sydney's DMD, you will have to order and pay for two separate DAT score reports.

MCAT Results

MCAT results: you can either print your MCAT report and mail, e-mail or fax it directly to OzTREKK, or you can simply e-mail your MCAT verification code & your AAMC ID number directly to OzTREKK.
